Netsa Contact + Upload + Zip + Mail is a web based application that will create any type on online forms with the ability to upload attachments, zip them on the server and send the documents and field information onto the desired mail addresses. The app has the following features.

Please note the examples on my server are set to 3 files maximum with a maximum combined upload size of 2MB.
